Family Needs Assessment
Our confidential assessment tool helps identify your family's most pressing needs and matches you with the WPFF programs best suited to your situation. The assessment takes approximately three to five minutes and generates personalized recommendations that our intake team uses to expedite your service plan development.
How the Assessment Works
The Family Needs Assessment evaluates your household situation across five interconnected domains that research consistently identifies as the primary drivers of family stability. For each domain, you will select the response that most closely matches your current circumstances. The assessment uses a weighted scoring system developed in consultation with our clinical advisory board to generate prioritized recommendations.
Upon completion, you will receive a detailed results page showing which WPFF programs are recommended for your family, ranked by urgency level. High-priority recommendations indicate areas where immediate intervention may be needed, while medium and standard recommendations reflect opportunities for preventive support and skill building.
Confidential Family Needs Assessment
This brief assessment helps our intake coordinators understand your family's current situation and match you with the most appropriate support programs. All responses are confidential and protected under our HIPAA-compliant data handling policies. The assessment takes approximately 3–5 minutes to complete.
What Happens After the Assessment?
Review Your Results
Your personalized recommendations appear immediately on screen. We encourage you to note which programs are flagged as high priority, as these will be addressed first during your intake meeting. You may also print or screenshot your results for reference.
Schedule an Intake Appointment
Call our intake line at (704) 555-0198 extension 110 or email [email protected] to schedule a 60-minute intake meeting. Intake appointments are available Monday through Friday from 9:00 AM to 4:00 PM and Saturday mornings by arrangement.
Meet Your Case Coordinator
During your intake meeting, a licensed case coordinator will review your assessment results, gather additional information about your family's circumstances, and develop a personalized service plan that addresses your identified needs with specific program enrollments, timelines, and goals.
Begin Receiving Services
Most families begin receiving services within one to two weeks of their intake appointment. Your case coordinator remains your primary point of contact throughout your enrollment, providing ongoing support, progress check-ins, and adjustments to your service plan as your family's needs evolve.
Assessment FAQ
Is the assessment confidential?
Yes. The assessment runs entirely in your web browser. No data is transmitted to our servers during or after completion. If you later contact us for intake, you may reference your results but they are not automatically sent to our team. All information shared during the formal intake process is protected under our HIPAA-compliant confidentiality policies.
Can I retake the assessment?
Absolutely. You may retake the assessment as many times as you like. Family circumstances change, and we encourage you to reassess periodically, especially if your situation has shifted since your last evaluation. There is a "Retake Assessment" button available after you view your results.
What if I need immediate crisis help?
If your family is in immediate danger or experiencing an acute crisis, please do not wait for assessment results. Call our main line at (704) 555-0198 and ask for the crisis coordinator, or contact the Mecklenburg County Crisis Line at (704) 566-3410. For life-threatening emergencies, always call 911 first.
Do I need to create an account?
No. The assessment requires no login, no account creation, and no personal information. It is completely anonymous. You provide identifying information only when you choose to contact us for intake services.
